How to Request for Smart Postpaid or Prepaid SIM Replacement
Smart subscribers who have registered their SIM can quickly request a SIM replacement if it is defective, lost, or stolen or if they want to upgrade to an LTE/5G SIM. SIM replacement cost is free.
Just visit the nearest Smart Store and bring 1 Valid ID as Proof of Identification.
The ID should have the following details:
- Full name
- Photo
- ID number
- Signature
- Issuing entity
Subscribers who still need to register their SIM may request a SIM replacement provided they submit the following documents and complete the SIM registration process in-store.
- Duly notarized Affidavit of Loss
- Valid Proof of Identification
- Present one 1 primary valid ID or two 2 secondary IDs
- Proof of SIM Ownership. Present any of the following:
- PUK Slip
- SIM Bed/original packaging of the SIM card
- Official Receipt with the subscriber’s mobile number details
- Successful log on to Smart App or My Smart Web
